Assistance with Discharge Plan

The best time to start planning for discharge is just after your family member is admitted. While it may seem too soon to think about going home, planning gives you more time to prepare.

Your family member should not leave the rehab facility until there is a safe and adequate discharge plan. This means that the plan meets your family member’s needs and that you can do what’s expected of you.

  1. Our Care Director can come to facility to communicate with the health care team to come up with best plan of home care services.
  2. Our Care Director will come in to facility meet with your loved one and family to discuss services we provide and answer questions.
  3. Assistance Home Safety recommendations and home set up and recommendations for supplies.
  4. Our Care Team will be set up and ready to care for your loved when they get home.
